Information Checklist
Some items you will need to complete your tax preparation are as follows:
- Your Social Security Card(s) and/or Your Spouse's
- Dependent(s) Social Security Numbers & Date of Birth(s)
- Wage Statements - Form W-2 or 1099-MISC
- Pension or Retirement Income - Form 1099-R, which are Distributions from Pensions, Retirement or Profit Sharing Plans, IRA's, Insurance Contracts, etc.
- Interest & Dividend Income - Form 1099-INT or 1099-DIV, if you do not receive such forms and still earn interest make sure you have your year end bank statements with you. They will show the interest you have earned over the year.
- State Income Tax Refund Amount - Form 1099-G, you will need this if you deducted this amount as an itemized deduction in an earlier year. This should be included as income.
- Social Security Income - Form SSA-1099, some of the social security benefits are taxable and should be stated on the form.
- Unemployment Income - Form 1099-G, this compensation is taxable and should be included as income on your tax return.
- Commissions Received/Paid
- Information on Sales of Stocks or Bonds - Form 1099-B, remember you will need the original dates bought and value as well as the sell dates and value. Short-term is defined as a stock or bond being held for less than a calendar year; whereas Long-term is defined as a stock or bond held for more than a calendar year.
- Self-Employed Business/Farm Income & Expenses - Form 1099-MISC, if you run a business or are a sole proprietor, you will need your accounting records showing your income totals and expense totals. If all you have are receipts and bank statements, get a ledger to get them organized, it will make things easier on you.
- Lottery or Gambling Winnings - Form W-2G
- Lottery or Gambling Losses, gambling losses can be deducted on your income tax return up to the amount of gambling winnings which must be documented by either a W-2G, WIN/LOSS Statment from the casinos or a gambling log.
- Income & Expense from Rentals
- Income from Partnerships, S Corporations, Trusts and Estates - Schedule K-1
- IRA Contributions
- Alimony Paid or Received, if you received alimony that is considered taxable income, whereas alimony paid is dedcutible on the tax return. Child support paymenst are not deductible or considered as taxable income.
- Child Care Expenses & Provider Information - Form W-10, if you have a child 13 or under and/or a disabled child of any age, the expenses incurred are deductible. The form must be completed for the deduction to be applicable.
- Medical, Eye Care, and Dental Expenses - if you itemize your deductions these expenses may be deductible
- Cash & Non-Cash Charitable Contributions
- Record of Purchase or Sale of Residence
- Mortgage or Home Equity Loan Interest Paid - Form 1098
- Real Estate and Personal Property Taxes Paid
- State or Local Sales Taxes Paid
- Unreimbursed Employment-Related Expenses
- Job-Related Educational Expenses
- Educator Expenses
- Tuition & Education Fees - Form 1098-T
- Student Loan Interest - Form 1098-E
- Casualty or Theft Losses
- Estimated Taxes - also known as quarterlies
- Foreign Taxes Paid - Form 1099-INT
